Terms of use

Below you'll find the terms of use implemented on Jelly form lite and Jelly form premium. Please read and follow the guidelines carefully:

  1. Creative commons license. Jelly form lite and Jelly form premium are licensed under the Creative Commons Attribution 3.0 License. This means, you may use this form for both non-commerical and commercial use. While we've made it legal to use Jelly form on commerical sites, a donation would be appreciated.
  2. As is packaging. Both versions of Jelly form come as is. That means, we are not responsible for any corruption of data, loss of goods or services, viruses, spyware or destruction of any property (hardware or software). While Jelly form has been extensively tested and is regularly checked for inconsistencies, there is still a possibility for manipulation of data.
  3. Form link. While it is not required, we recommend and appreciate if you keep a link (such as one of the sample form), at the end of the form using the Jelly form source.
  4. Copyright of materials. Jelly form lite and Jelly form premium are the sole property of Jelly form/Avenue Designers/Shane Holland. We reserve the right to manipulate the form's code at any given time without prior notice. All logos, images, source code and downloadable files contained on jellyform.com are exclusive to Jelly form.
  5. Not required donation. Although Jelly form is free, we ask you help support the future and present development of Jelly form lite and Jelly form premium. While a donation is not required, we appreciate any monetary amount if you're feeling generous. For more information on how to donate, see our donation page.
  6. Acceptance of terms. If you hereby accept the following terms above, you are permitted to use Jelly form lite, and Jelly form premium (account required).
