Embed the feedback widget on your site

Add the JellyForm widget to your own website so visitors can submit feedback without leaving the page.

The feedback widget lets visitors submit anonymous feedback directly on your own website — no need to send them to your JellyForm link. You configure it per box in the Embed tab, copy a small code snippet, and paste it into your site's HTML.

Enable the widget

  1. Open your box and click the Embed tab.
  2. Switch on Enable Widget Embed.
  3. Click Save Changes. The widget only loads on pages that include your embed code while this toggle is on.

Choose an embed mode

  • Floating — a button sits in the corner of the page; clicking it expands the feedback form.
  • Inline — the form renders directly inside your page, wherever you place the snippet.

Customize the launcher button

In Floating mode, the Launcher Button section controls the button itself. Launcher customization is available on Team and Business plans.

  • Position — Bottom Left or Bottom Right.
  • Launcher Text — up to 30 characters; leave it blank for an icon-only button.
  • Launcher Icon — Chat, Feedback, Lightbulb, Heart, or Megaphone.

Behavior settings

  • Auto-open after delay — opens the widget automatically after 1–60 seconds. Available on Team and Business plans.
  • Close after submission — closes the widget panel once feedback is submitted.
  • Show conversation link — shows the respondent their private conversation link after submitting, so they can check back for replies. See The private conversation link.

Add the code to your site

Copy the snippet from the Embed Code section — it updates automatically to match your chosen mode — and paste it into your website's HTML. For Inline mode, place it where you want the form to appear.

If your site uses a Content Security Policy, add jellyform.com to your script-src and connect-src directives, plus fonts.googleapis.com (style-src) and fonts.gstatic.com (font-src) for the widget font. The Embed tab shows this notice next to the code.

Restrict which sites can embed your widget

In the Allowed Domains section, list the domains where your widgets may load — one per line, like app.yourcompany.com. When the list is empty, the widget works on any site; once you add domains, requests from anywhere else are refused. The list applies to every box in your organization. Available on the Business plan.

Prefer sharing a link instead? See Share your box: link, QR code & printable flyer. The widget picks up your logo and accent color too — see Logo, accent color & hiding JellyForm branding.
